The Michigan's Public Health System is asking for your help.  Please take a moment to complete the Michigan State Health Assessment Survey for people who live across Michigan. This anonymous survey asks Michiganders what could make their community a healthier place to live.  Survey details include:

• Purpose: Capture responses from Michiganders from across all of Michigan to identify the greatest health needs that currently exist in the state.
   
• Time frame: We are conducting the survey from June 1, 2020 to June 30, 2020.
